Problems and Issues Register Template
The Problems and Issues Register is a Word template for logging problems and issues as they arise, with the date, who logged them, the area or process affected, the details and the action taken.
What's Included in this Register
- Date and Logged By - when the problem was raised and who recorded it
- Area / Process - the part of the business affected
- Details of Problem / Issue - what happened
- Action Taken - what was done to put it right
Built for Nonconformity and Corrective Action
- Gives a running record of problems that can be reviewed for trends
- Supports the control of nonconforming outputs and corrective action in ISO 9001
- Links each problem to the action taken
Who Needs a Problems and Issues Register?
Any organisation that wants a simple log of problems and issues, and those working to ISO 9001, ISO 14001 or ISO 45001 that need to show problems are recorded, acted on and reviewed.
Using the Register
Encourage everyone to report problems and log each one with what was done. Review the register regularly, for example monthly or at management review, to spot trends across areas and processes and to check that the actions taken have worked.
What to Log
The register is for any problem or issue, large or small, that affects the way the business runs. Typical entries include:
- Mistakes or delays in a process, or work that has to be redone
- Late, damaged or incorrect deliveries from suppliers
- Equipment faults and breakdowns
- Documents or information found to be out of date
- Issues found during inspections, checks or maintenance
- Issues raised by staff, by customers or at internal audit
- Customer complaints, unless they are logged separately on the F-IMS51 Customer Complaints Register
From Problem to Root Cause
Most entries are closed by the action taken at the time. Where a problem is significant or keeps coming back, record the investigation on the F-Q10 Significant Problem, Incident and Complaint Form or the root cause analysis on the F-IMS37 Root Cause Analysis Review Register, so the register stays a quick log and the detail is kept where it belongs.
Document Control
The footer carries the register number and title, issue, information classification and page numbering. The register can be listed with your other controlled documents on the F-IMS20 Document Register.
Works with the Issues and Actions Register
